At Guided Ability, we provide Medicaid-funded services to individuals with intellectual and developmental disabilities, primarily Attendant Care in Lane County, Oregon. We may also be able to provide other services in surrounding areas depending on individual needs and availability.
We support people in building lives that feel more independent, connected, and aligned with what matters to them. Our Direct Support Professionals (DSPs) provide services that are individualized and based on each person's goals, routines, and preferences. No two people are alike, so no two support plans are the same.

Community Participation
Support with getting out into the community, attending events, volunteering, and finding opportunities in line with people's interests.

Social Skills and Relationship Building
Practicing communication, building friendships, navigating social situation, and developing confidence in connecting with others.

Money Management and Financial Skills
Help with budgeting, paying bills, grocery shopping, and understanding everyday financial decisions.

Housing Support
Assistance with searching for housing, completing applications, communicating with landlords, and maintaining a safe and stable home.

Activities of Daily Living
Support with cooking, cleaning, laundry, organizing, and creating routines that make daily life more manageable.

Personal Care and Hygiene
Respectful support with bathing, toileting, dressing, grooming, and other personal care needs, always with attention to dignity and choice.

Medical and Healthcare Support
Help with organizing and keeping track of medical appointments, medication reminders and management, and providing support before, during, and after appointments so individuals feel prepared, understood and comfortable.

Personal Training and Nutrition Coaching
For individuals interested in fitness and overall health, one-on-one personal training and nutrition support may be available. These services are provided directly by the owner, who is NASM-certified, and are offered at a limited capacity.

Transportation
Help with getting to appointments, community activities, and running errands.
* All services are provided in alignment with each individual’s authorized Individual Support Plan (ISP) and available funding, which may determine the type and level of support provided.
As a Standard Model Agency, we hold a higher standard for the quality of services we provide. This includes clear and consistent documentation, active follow-through on ISP goals, and thoughtful communication across the support team. We also work to maintain reliable staffing and provide as much consistency and coverage as possible, while being transparent and proactive when adjustments are needed.
As a smaller company, we are able to stay more involved and have the capacity to get to know you on a deeper level. We take time to get to know each person, make thoughtful matches, and adjust support when something is not working. We understand that receiving support requires a level of trust and vulnerability. Because of that, we will never send someone into your home without first making a meaningful introduction.
